To ensure accuracy in our analysis and prompt filing of all the relevant forms in order for you to receive your refund, please gather all the required reports and documents and submit them in the form below. Attach the reports in the respective areas.
Please prepare these documents before you start:
- 1Profit and Loss statement for all 2019, 2020 and 2021 - Q1 to Q3.
- 2Quarterly tax returns for all 2020 and 2021 - Q1 to Q3
- 3Payroll register for 2020 (3.13.2020 to 3.31.2020, Q2, Q3 and Q4), 2021 (Q1 to Q3) showing all wages and tax breakdown by employee by quarter
- 4Medical health benefits (employer portion) by month for all relevant periods
- 5PPP application
- 6PPP forgiveness letters (if applicable)
