  • Coverage is available for your spouse and eligible dependent children (with most plans).
  • Benefits are paid directly to you, unless you specify otherwise.

Disability Insurance

Average cost: $10.57/week

Provides income replacement to help make ends meet, if you are unable to work due to a covered injury or sickness. This coverage helps you maintain your lifestyle.

On/Off job and world wide coverage

California offers up to 55% of your annual income when claim disability, Colonial Life will add on up to 40%

Policyholder will be paid regardless of any insurance they may have with other companies, when they file a claim.

Accident Insurance

Average cost: $5.92/week

Helps offset the unexpected medical expenses such as deductibles, co- payments, transportation and emergency room fees that can result from a fracture, dislocation or other covered accidental injury.

On/Off job and worldwide coverage

Different Levels: Basic, Preferred, and Premium

Health Screening Benefits - $50 per covered person per calendar year

Can provide you with benefits for a wide range of situations including Initial care, surgery, transportation and lodging, and follow-up care.

Cancer Insurance

Average cost: $8.12/week

Helps offset the out-of-pocket medical and indirect expenses related to cancer which most medical plans may not cover (estimated to be 65% of the total cost of treatment).

Cancer insurance provides benefits for specified cancer screening tests and a wellness benefit that you can use every year

It can help pay for necessities such as Car at out-of-network specialty centers, home health care, experimental treatments, lost income, child care, travel, lodging and meals during treatment.

Critical Illness Insurance

Average cost: $5.92/week

This coverage complements your major medical coverage by providing a lump sum benefit you can use to help pay the direct and indirect costs related to a covered critical illness such as heart attack, stroke, renal failure, organ transplant and coronary artery surgery, which can be lengthy and expensive.

You may use this coverage more than once for both the same Specified Critical Illness and a different Specific Critical Illness.

Health Screening Benefits including 21 tests such as stress test on a bicycle or treadmill, echocardiogram, chest x-ray, and colonoscopy, to name a few.

Life Insurance

Average cost of $4.44 per week

Life Insurance enables you to tailor coverage to your individual needs and help provide financial security for your family members. As your needs change over time, you can update your coverage to best-fit life changes.

Whole Life 1000 is an individual whole life insurance plan that offers a guaranteed level premium, guaranteed cash value and guaranteed death benefit. Coverage remains in force during the insured's entire lifetime (policy endows at age 100), provided premiums are paid as specified in the policy.

Term Life Insurance is an individual term life insurance plan that offers three term periods, level death benefits and guaranteed premiums, and is guaranteed renewable to age 95, convertible to age 75 and portable. It can be sold through payroll-deduction or individually.

Universal Life 1000 is an individual flexible premium, adjustable death benefit policy available for payroll-deduction and non-payroll sales. Coverage remains in-force for the life of the policy provided cash value is present.

Dental Insurance

Average cost: $8.82/week

Dental insurance policy provides the same benefit amounts with any dentist you choose and covers a variety of dental procedures, from routine cleanings to more advanced procedures such as crowns and root canals. Additional savings are available by visiting a network dentist.

It can be used at both out-of-network and in-network dentists

Hospital Confinement Indemnity Insurance

Average cost: $5.01/week

This policy provides a lump-sum benefit for a covered hospital confinement or a covered outpatient surgery to help with co-payments and deductibles that are not covered by most major medical plans.
