Payroll Set Up Forms

This payroll set up forms section is divided in to two sections: First is the Employer Set Up for new employers starting out with our payroll service. The second section is the Employee Set Up forms. These forms could be used to set up your employees if you are starting your payroll service with us or are an existing client adding new employees to your payroll service.

Employer Set Up

  • NEW CLIENTS: The above set of 3 documents is required to set up your account.
  • FORM 8655: This form authorizes AccuPay to efile your tax returns
  • NOTE: Please  do not email your documents to AccuPay. Upload them to our secure site
